Heartland Healthcare began its journey in 2008 as Heartland Home Health, with a mission to empower seniors to maintain independence by providing high-quality home health services. In 2019, a change in ownership marked a turning point, fueling organizational growth and the expansion of services. Recognizing unmet needs within the geriatric population across western South Dakota, Heartland responded by adding hospice care—becoming Heartland Home Health and Hospice—to ensure compassionate, comprehensive end-of-life care was accessible to all.
Further expanding its reach, the organization acquired an assisted living facility in Spearfish, now known as Heartland Senior Living, enabling a broader continuum of care. As its services grew more comprehensive—spanning home health, hospice, and assisted living—so too did its identity, evolving into Heartland Healthcare.
